PRP for skin and hair

Also known as: vampire facial, PRP injections, platelet-rich plasma, autologous platelet-rich plasma

PRP concentrates platelets from your own blood and injects or microneedles them into skin or scalp, delivering a payload of growth factors. The strongest evidence is for pattern hair loss, where multiple meta-analyses of RCTs show significant hair density gains. For facial rejuvenation the story is weaker: a systematic appraisal of 36 studies covering 3,172 patients concluded the clinical evidence is very limited, largely because preparation methods vary wildly between studies and clinics. The vampire facial hype exceeds the facial data considerably.

How it works

Blood is drawn and centrifuged to concentrate platelets 2 to 6 times above baseline. When activated, platelets release growth factors (PDGF, VEGF, TGF-beta and others) that stimulate fibroblasts, collagen production, blood vessel formation, and, in the scalp, appear to prolong the hair follicle growth phase. Because it is your own blood, allergy risk is essentially nil.

What the evidence says, claim by claim

These are the results measured in studies, not the results shown in ads. Where the research is weak, the grade says so.

For hair thinning and loss

Moderate evidence

A 2023 systematic review and meta-analysis of RCTs found PRP significantly increased hair density versus placebo at 3 and 6 months, and a 2024 meta-analysis of randomized trials estimated a mean gain of 27.6 hairs per square centimeter (95 percent CI 14.0 to 41.1). Typical protocols use 3 monthly injections plus periodic boosters. Limitations are honest ones: high heterogeneity between studies, unstandardized PRP preparations, and evidence of publication bias, so the grade stops at moderate.

Randomized trials support this use, but they are few, small, or short. The effect is probably real. The size of it is less certain.

For wrinkles and fine lines

Limited evidence

A 2021 systematic appraisal of 36 studies (3,172 patients) found the clinical evidence for PRP facial rejuvenation very limited, dominated by uncontrolled studies with inconsistent PRP preparation. A 2025 systematic review of 11 studies reported improvements in wrinkles, texture, and hydration in several trials, but sample sizes stay small and blinding is rare. Plausible modest benefit, unproven against standard treatments.

Only small, short, or lower-quality studies exist. Treat marketing claims about this use with caution.

For acne scars (as an add-on)

Moderate evidence

PRP's best facial use is as an adjunct: a 2022 systematic review and meta-analysis found adding PRP to ablative fractional CO2 laser improved atrophic acne scar outcomes and shortened downtime versus laser alone. Evidence for PRP by itself on scars is much weaker; the value is in combination with resurfacing or microneedling.

Randomized trials support this use, but they are few, small, or short. The effect is probably real. The size of it is less certain.

For dark circles

Not enough evidence

Under-eye PRP is heavily promoted, but supporting data amounts to small split-face and comparative trials, such as a 2021 randomized split-face study of injection techniques for periorbital hyperpigmentation, and in one comparative RCT more than half of PRP-treated patients were nonresponders for infraorbital dark circles. There is no meta-analytic support; treat marketing claims here with skepticism.

The marketing has moved faster than the research. There is no good human data behind this use yet.

Risks and what can go wrong

Downtime is 1 to 3 days of swelling, redness, and bruising at injection sites; under-eye injections bruise most. Because PRP is autologous, allergic reaction risk is minimal, but sterile technique matters (a widely reported 2018 incident involved infections from contaminated equipment at a spa, not the PRP itself). Avoid with platelet disorders, active anticoagulation issues, blood cancers, active infection, and pregnancy as a precaution. Safe across all skin types.

The practical details

Typically 3 sessions spaced 4 weeks apart, then boosters every 6 to 12 months (scalp protocols are similar). US pricing runs roughly 500 to 1,500 dollars per session for scalp or face. Strictly in-office with a blood draw each visit; results depend on the clinic's preparation system, which is a real and underappreciated source of variability.
